Purpose. Used by a work team to break a task into steps, identify the hazards at each step and agree the controls before the task starts.
1.When to prepare a job safety analysis
A job safety analysis must be prepared for any task that does not have a current procedure or work instruction, for any task being done for the first time, for any task that a permit to work requires and for any task where the supervisor identifies a hazard that is not covered by existing controls.
2.Who prepares it
The people who will do the task must prepare the analysis with their supervisor. Everyone who will take part must read and sign it before starting. A contractor is permitted to use their own format provided it covers the same content and is reviewed by the Apex permit issuer.
3.Breaking down the task
List the steps of the task in the order they will be done. Keep each step to a single action. For each step identify what could go wrong and who could be harmed, then record the control using the hierarchy of controls, preferring elimination, substitution, isolation and engineering over administrative controls and personal protective equipment.

4.Rating the risk
Rate the risk of each step before and after controls using the risk matrix in the risk assessment procedure. Any step that remains at high or extreme after controls must be referred to the department manager and must not proceed.
5.Review and filing
The supervisor must review the analysis, confirm the controls are in place and sign it. Completed analyses must be attached to the permit where one applies and filed with the department records. If conditions change during the task, work must stop and the analysis must be updated.
